Built around your work

Know what matters before you reply.

An inquiry that says only “I need help” leaves your team starting from scratch. A long questionnaire can ask for too much too soon. Choose the questions that help you understand the request and respond thoughtfully.

Choose the format that fits the conversation.

Prepare a service inquiry

Ask about the service, location and timing so the person responding can prepare for the conversation.

Learn from customers

Use a short survey after a completed service to understand what worked and what needs attention.

Guide a first decision

Offer a scored quiz that helps someone identify a starting point, then invite them to discuss the result.

Forms, Surveys & Quizzes

Make each question earn its place.

Collect enough context to be helpful, with a format and follow-up that fit the task.

Focused fieldsCapture details your team can use.

Add contact details and business-specific fields such as service type or preferred timing. Give each field a clear purpose. Review required fields and contact-matching settings before publishing.

Conditional questionsKeep the next question relevant.

Show or hide fields based on answers. Survey branching can send respondents to another slide instead of asking everyone the same questions. The current survey logic requires a newly created or cloned survey; test every path before launch.

Scored quiz resultsHelp visitors understand their starting point.

Assign points and categories to answers, then explain the result with tailored text and a next-step link. Your team defines what each result means. A score is guidance based on your rules, not an independent assessment.

Contact record mappingKeep quiz outcomes available for follow-up.

Enable the quiz results you want saved to contact fields, such as overall score, tier or category. Those fields can support contact filtering and configured workflows. A repeat submission replaces the saved values for that contact and quiz.

Sharing and embeddingMeet visitors where they already are.

Share a direct form or quiz link, or embed it on a compatible website or funnel page. Adapt the styling to your brand and test the published experience on a phone as well as a desktop.

Confirmation and follow-upGive the response a clear owner.

Configure a confirmation message, email acknowledgment or internal notification as appropriate. A submission trigger can start a planned workflow. Check existing notifications first so the same request does not produce duplicate messages.

A better estimate request

A home-service company asks which service is needed and where the property is located. Relevant follow-up questions appear based on the answer. The office reviews the submission before confirming whether it can help.

Questions before you start

Should we use a form, survey or quiz?

Use a form for a focused request, a survey for a sequence of questions or feedback, and a quiz when answers should produce a scored or category-based result. Start with the decision the information will support.

Can we show different questions based on answers?

Yes, through configured conditional rules. Current survey branching requires a newly created or cloned survey. These survey rules are not included in account snapshots, so verify them after a transfer as well as before launch.

Do quiz results save to the CRM automatically?

Enable the result fields you want to sync in the quiz settings. On submission, the selected outcomes are written to contact fields. Retaking the same quiz replaces those saved result values; do not treat them as a history of every attempt.

Can we keep our existing website?

Yes, forms and quizzes can be shared by link or embedded on compatible external websites. Check the site’s embed permissions, mobile layout and submission behavior before sharing the page.

Will every response create a new contact?

A new lead can create a contact record. Matching and duplicate behavior depend on account settings. Test an existing contact as well as a new one so repeated submissions behave as intended.

Can the team receive a notification?

Yes, internal email notifications can be configured. More involved follow-up uses a configured workflow. Confirm the recipients, sender settings and existing automations to avoid duplicate acknowledgments or alerts.

Can we collect sensitive information?

Only after the intended data, access controls and applicable requirements have been reviewed. Ask for the minimum information needed. A form or consent checkbox alone does not establish that a sensitive-data process is appropriate.

Does setup include ongoing follow-up?

Only when it is part of the agreed service. We can help define the form and response process, while your team or a separately scoped service owns reviewing answers and continuing the conversation.
